Specific Prohibited Uses.
The Collom & Carney Clinic Association Web Site may be used only for lawful purposes by individuals interested in services provided by Collom & Carney Clinic Association. Collom & Carney Clinic Association specifically prohibits any use of the Web Site, and all users agree not to use the Web Site, for any of the following:

User Submissions
Collom & Carney Clinic Association acts as a passive conduit for the online distribution and publication of user-submitted information and has no obligation to screen communications or information in advance and is not responsible for screening or monitoring material posted by users. If notified by a user of communications which allegedly do not conform to these Terms and Conditions, Collom & Carney Clinic Association may investigate the allegation and determine in good faith and its sole discretion whether to remove or request the removal of the communication. Collom & Carney Clinic Association has no liability or responsibility to users for performance or nonperformance of such activities. Collom & Carney Clinic Association reserves the right to expel users and prevent their further access to the Web Site for violating the Terms and Conditions or the law and the right to remove communications which are abusive, illegal, or disruptive. Refund Policy
Upon notification to business office of a duplicate or erroneous transaction to patient’s credit card the following steps are taken:

  • Verification of duplicate or erroneous transaction against credit card receipt vs cashier’s edit by business office staff..
  • Notification to IT that the duplicate or erroneous transaction needs to be removed including date posted on credit        card and total amount
  • Duplicate or erroneous transaction will be cancelled on the cashier’s edit
  • Notification to patient of correction of duplicate or erroneous transaction to credit card.
